The Camp Health Form is required for all campers at full-day and residential camps. Click here for the health form.
The Camp Authorization Form is required only for campers where medication is left on-site with our staff.
Click here for the medical authorization form – Note this one can be filled out on your screen and then you can print and sign it. Or if you prefer you can save and/or print out a blank copy and complete it off-line.
